Our licensed clinicians speak directly with consumers about any challenges related to medication availability or usage of prescribed medications. We also discuss the importance of other preventive health screening tests that could help improve consumer health. We often help coordinate these tests with a consumer’s doctor or local clinics. In many cases, our clinicians can offer delivery of medications to the consumer’s home after reviewing the best pharmacy options.
AdhereHealth works on your health plan’s behalf to make sure nothing is preventing you from accessing the best care or following your care plan, from transportation to your physician to the high cost of medications or problems with medication delivery. We call after a non-emergency issue has been identified and needs to be resolved. This may involve coordination among your providers and favorite pharmacy.
AdhereHealth provides two types of letters: a Medication Therapy Management (MTM) offer letter or Medication Review Follow-up Letter.
Your health plan is offering this free medication review to make sure you’re receiving the best care possible. Since the average consumer sees five or more providers and visits multiple pharmacies, it’s likely at least one medication-related issue exists (this is true in 90% of all reviews). During your confidential AdhereHealth review, a licensed pharmacist helps you recall all medications, including over-the-counter, so nothing is missed that could affect your health.
AdhereHealth only calls when a confidential review of your medical history shows at least one non-emergency issue that, once resolved, could improve your care. Once resolved, your name is automatically removed from our call list. While we attempt to address all care issues during one call, multiple calls may be needed.
