Schedule a Medication Review with a Pharmacist

This is a Medication Therapy Management (MTM) program that provides education and information about your medicines. We are offering you the opportunity to review your medications over the telephone with a pharmacist to help make sure you are on the right medications and receiving the best possible care. Participation in this program is voluntary and available at no cost to you.

During the review, we will try to answer three main questions:

1. Are you on the best medicines for your medical conditions?
2. Are the medicines being used correctly?
3. Are there options for your medicines that will save you money?

After the medication review, we will mail you a medication action plan and medication list. This information is to help you when you talk to your doctors. Your doctor will help you decide if our suggestions are right for you.
