This is a Medication Therapy Management (MTM) program that provides education and guidance about your medications. We offer you the opportunity to review your medications over the phone with a pharmacist to help ensure you are taking the right medications and receiving the best possible care. Participation in this program is voluntary and available at no cost to you.
During the review, we will focus on three key questions:
- Are you taking the best medications for your medical conditions?
- Are your medications being used correctly?
- Are there lower-cost options available for your medications?
After your medication review, we will mail you a personalized medication action plan and a complete medication list. This information is designed to support conversations with your doctors, who will help determine whether any recommended changes are right for you.
